Pepto-Bismol Original Liquid
Relieves stomach discomfort for those who enjoy the original Pepto-Bismol flavor.
A liquid formula with a wintergreen taste that relieves heartburn, indigestion, upset stomach, nausea, and diarrhea. Available in 4-, 8-, 12-, and 16-ounce bottles.
Drug Facts
Active Ingredient (in each 15 ml Tablespoon): Bismuth subsalicylate 262 mg
Purpose: upset stomach reliever and antidiarrheal

Warnings
Reye's syndrome: Children and teenagers who have or are recovering from chicken pox or flu-like symptoms should not use this product. When using this product, if changes in behavior with nausea or vomiting occur, consult a doctor because these symptoms could be an early sign of Reye's syndrome, a rare but serious illness.
Allergy alert: Contains salicylate. Do not take if you are:
• Allergic to salicylates (including aspirin)
• Taking other salicylate products
Do not use if you have
• An ulcer
• A bleeding problem
• Bloody or black stool
Ask a doctor before use if you have
• Fever
• Mucus in the stool
Ask a doctor or pharmacist before use if you are taking any drug for
• Anticoagulation (thinning the blood)
• Diabetes
• Gout
• Arthritis
When using this product a temporary, but harmless, darkening of the stool and/or tongue may occur.
Stop use and ask a doctor if
• Symptoms get worse
• Ringing in the ears or loss of hearing occurs
• Diarrhea lasts more than 2 days
If pregnant or breast-feeding, ask a health professional before use.
Keep out of reach of children. In case of overdose, get medical help or contact a Poison Control Center right away.
Directions
• Shake well before use
• For accurate dosing, use dose cup
• Adults and children 12 years and over:
1 dose (2 Tbsp or 30 ml) every ½ to 1 hour as needed
• Do not exceed 8 doses (16 Tbsp or 240 ml) in 24 hours
• Use until diarrhea stops but not more than 2 days
• Children under 12 years: ask a doctor
• Drink plenty of clear fluids to help prevent dehydration caused by diarrhea

【適応】下痢症
【禁忌】
(1)出血性大腸炎の患者 [腸管出血性大腸菌 (O157等) や赤痢菌等の重篤な細菌性下痢患者では, 症状の悪化, 治療期間の延長を来すおそれがある]
(2)慢性消化管通過障害又は重篤な消化管潰瘍のある患者 [ビスマスの吸収による副作用が起こるおそれがある]
【原則禁忌】細菌性下痢患者 [治療期間の延長を来すおそれがある]
【慎重投与】便秘の患者, 結腸ろう造設術, 回腸ろう造設術又は人工肛門造設術を受けた患者, 消化管憩室のある患者 [ビスマスが吸収され, 重大な副作用が起こるおそれがある]
【薬効薬理】収れん並びに粘膜面, 潰瘍面を被覆保護する作用を有し, また腸内異常発酵によって生じる硫化水素と結合するため, 胃腸カタル, 胃痛, 潰瘍, 下痢などに効果があるとされる。また内服により腸内に生じた硝酸イオンは, 大腸菌により還元されて徐々に亜硝酸イオンとなり, 血管拡張, 血圧降下作用が認められる (Stieglitz, 1936)

Reye症候群
定義: 肝障害を伴う急性脳症
病因: インフルエンザや水痘患児にアスピリンを投与すると発症率増加. 解熱薬としてアスピリンを小児に投与することは禁忌.
病理:
①肝臓の脂肪変性 (急性肝不全)
②ミトコンドリア障害
③強い脳浮腫 (急性脳症)
好発年齢: 1~3戚
症状:
先行するウイルス感染 (発熱)
→嘔吐, 不機嫌, けいれん, 意識障害が出現
肝障害があるが, 黄疸はない.
検査:
① AST↑, ALT↑, LDH↑. ただし, 黄疸なし.
② アンモニア↑
③ 血糖↓
④ プロトロンビン時間延長
⑤ 髄液: 圧↑↑, 糖↓. ただし, 細胞数は正常
治療:
①脳浮腫の改善: グリセオール®, マンニトール,過換気
②肝障害: ビタミンK投与
予後: 40% は死亡.

急性胆嚢炎の診断基準
A. 右季肋部痛(心窩部痛),圧痛,筋性防御,Murphy sign
B. 発熱,白血球数またはCRPの上昇
C. 急性胆嚢炎の特徴的画像検査所見
疑診: AのいずれかならびにBのいずれかを認めるもの.
確診: 上記疑診に加え, Cを確認したもの.
ただし, 急性肝炎や他の急性腹症, 慢性胆嚢炎が除外できるものとする.

• sonographic Murphy sign (超音波プローブによる胆嚢圧迫による疼痛),
• 胆嚢壁肥厚 (>4mm)
• 胆嚢腫大 (長軸径 >8cm, 短軸径 >4cm)

• デブリエコー
• 胆嚢周囲液体貯留
• 胆嚢壁sonolucent layer
• 不整な多層構造を呈する低エコー帯
• ドプラシグナル。
CT:
• 胆嚢壁肥厚
• 胆嚢周囲液体貯留
• 胆嚢腫大
• 胆嚢周囲脂肪織内の線状高吸収域。
MRI:
• 胆嚢結石
• pericholecystic high signal
• 胆嚢腫大

胆道シンチグラフィー (HIDA scan: technetium hepatobiliary iminodiacetic acid scan):
急性胆嚢炎の診断に有用な検査のひとつであるが, 本邦ではあまり用いられていない.

ERCP
適応:
• 閉塞性黄疸をきたす疾患の鑑別診断
• 膵疾患: 急性膵炎, 慢性膵炎, 膵嚢胞症, 膵腫瘍
• 胆道疾患: 胆嚢結石, 胆嚢隆起性病変(胆嚢cholesterol polypなど), 胆嚢adenomyomatosis,胆嚢癌, 胆管結石, 胆管癌など
禁忌:
• 心臓・肺・肝臓・腎臓などに基礎疾患・合併症がある場合
• 造影剤アレルギー
• 胃切除後は技術的に困難となる場合あり
• 治療中・治療後の安静が守られない
合併症:
• 急性膵炎 (まれに重症急性膵炎)
• 胆道感染症
• 腸管 (十二指腸)・胆管膵管損傷
• 薬物アレルギ―

Charcotの3徴: 腹痛・発熱・黄疸.
Raynoldsの5徴: Charcotの3徴に加えて, 意識障害・ショック.
壊疽gangrene: 疾患あるいは血行障害によって壊死に陥った組織が腐敗性変化を起こし汚穢褐色, ないしは黒色に変色した状態.
